LPN Outpatient Medication Clinic (Sanford Office)

Job Description:

Title:  LPN Outpatient Medication Clinic (Sanford Office)
 
Schedule:  Monday – Friday 7:30am-3:30pm
 
Fulltime Benefit Eligible position
 
DUTIES:                               
 
This position entails a high degree of confidentiality and detail orientation.  This position performs routine work in the care and rehabilitation of emotionally and psychiatrically impaired clients who receive services through the Outpatient Med Clinic.  Individual must be able to serve as a role model for appropriate behavior, communicate effectively and participate in verbal and de-escalation techniques when needed.  Ability to excel in a stressful, constantly changing environment as part of a team is required.  The Medical Care Manager supports the designated Team Leader Dr/ARNP (Provider) in an effort to supply quality care for the consumer.  The ability to perform both administrative and clinical functions in the clinic environment is fundamental to the requirements of this position.
 
1.       Responsible for opening the Medication Evaluation/Psychiatric Evaluation, measuring and recording vital signs (including temperature, pulse, blood pressure, weight and height), saving and placing document on the Providers homepage for all Medication Evaluation and Psychiatric Evaluation visits...
 
2.       Responsible for preparing consumer for Provider and performing any routine screening tests and recording results prior to session with Provider.
 
3.      Ensures that the Treatment Plan is complete and signed by the consumer no later than the next visit after the Treatment Plan Review is completed.
 
4.      Ensures that all required documentation is up to date, this is to be reviewed at each visit including med checks, clinic visits and Clozaril labs.  These documents include but are not limited to Mental Health Outcomes (due every 3 months), FARs (due every 6 months), PCP release (due annually), Psychiatric Evaluation (due annually).  The Receptionist must be notified that the next appointment needs to be for Psychiatric Evaluation Update.
 
5.       Ensures that a progress note is accurately completed for each visit.  All the billing elements of the progress note must be in real time with no overlapping service times and must be completed by close of business on the day service is provided.
 
6.       Ensures that Medication Informed Consents are completed and signed by the consumer as directed by the Provider.
 
7.       Responsible for completing Lab requests and processing as needed.
 
8.       Responsible for communications with Pharmacies as required.
 
9.       Responsible for copying all prescriptions, ensuring that consumer receives the prescription promptly, and forwards copy for scanning in EHR.
 
10.   Responsible for administration of medications as needed and/or ordered by Provider, and maintaining PAP and Sample medications as required.
 
11.   Responsible for disposing of biohazard waste according to OSHA standards.
 
ADDITIONAL DUTIES AS REQUIRED:  (This area must be initialed if part of job description).
a.       Responsible for appointment reminder calls to consumers and documenting calls as required by supervisor. ________ (Initials)
 
b.      Responsible for PCP voicemail box.  Retrieving calls from the PCP VMB, ensuring that a PCP release for this physician has been completed and signed by the consumer, documenting in an informational note (subject line PCP Consult) in the EHR and forwarding to Provider for follow up.               ________ (Initials).
 
12.   Employee will be aware of any changes in the Anasazi software and be responsible for acquiring any additional training in regards to system changes related to duties as described above.
 
13.   Employee will complete trainings, attend and/or participate in meetings as required by SBH and/or regulatory agencies to ensure compliance with mandates, and to maintain certifications and licensure.
 
14.   Employee will maintain the integrity of the clinical record through timely and accurate completion of all required paperwork.  Documentation required is program specific and may include, but is not limited to, treatment plans, assessments, psychosocials, SALs, progress notes, tracking forms, and intra-agency referrals. Employee shall submit paperwork to appropriate departments as required.
 
15.   Employee is responsible for completion of all required paperwork and training in order to receive a computer log-in to gain access to communication mediums used in conjunction with agency business.
 
16.   Employee will possess at a minimum, basic computer literacy skills needed to utilize all communication mediums used in the conjunction with agency business to include but not limited to; agency intranet, on-line computer training, agency email, applicable software programs.
 
17.   Employee will set up, maintain and use daily (where applicable) the following communication tools to keep abreast of agency updates to include but not limited to; intranet, agency voice mail, and agency email.
 
18.   Many clients are in severe emotional distress and employee must be able to communicate effectively and participate in both verbal and physical de-escalation techniques.  Employee must demonstrate ability to excel in a stressful, constantly changing environment as required. 
 
19.   Adhere to all Agency Policies & Procedures.
 
20.   Perform other duties as assigned by supervisor.
 
QUALIFICATIONS:
     
Licensed Practical Nurse required with a minimum of one year of experience in physician office or clinic setting with psychiatric care experience preferred.  Experience with Electronic Health Records or competent computer skills required.

Registered Nurse - Per Diem

Shift Differentials for Overnights/Weekends

Evening Shift 3pm-11pm - 5%

Overnight Shift 11pm-7am- 10%

Weekends

(beginning 7am-3pm shift on Saturday)

(last shift included 11pm-7am ending Mon am)- 15%

All direct care staff must attain DCF Clearance prior to working. SBH pays live scan screening cost which is the 1st step in the process.

PROGRAM: Crisis Stabilization Unit

DUTIES: This is a professional nursing position on the Crisis Stabilization Unit, a secure adult psychiatric facility. Many clients are in severe emotional distress and employee must be able to communicate effectively and participate in both verbal and physical de-escalation techniques. Ability to excel in a stressful, constantly changing environment is required. This person is responsible for all issues related to patient care. Varying responsibilities may include the administration and ordering of medications and supplies, providing clinical assessments, staff supervision, program facilitation and other direct care concerns. Assigned duties would include either medication nurse or charge nurse responsibilities.

QUALIFICATIONS: Graduation from an accredited school of nursing and licensed by the State of Florida as a Registered Nurse. Able to maintain licensure without restrictions. Previous experience with target population preferred.
